Mediastinal sarcoid-like reaction in cancer patients.

چکیده
Sarcoidosis is a multisystem disease of unknown origin characterized by the presence of noncaseating epithelioid cell granulomas. When these granulomas develop in the lymph nodes, and the patient does not meet criteria for the diagnosis of systemic sarcoidosis we refer to this as a sarcoid-like reaction. Because of the clinical implications of the diagnosis of lymph node metastasis, mediastinoscopy plays a key role in the differential diagnosis in these patients. We conducted a retrospective chart review of all patients who had undergone mediastinoscopy in our hospital from 2010 to 2015 with a diagnostic of sarcoidosis or sarcoid-like reaction and history of solid organ malignancy. Seventy-four mediastinoscopies were performed by three different surgeons. There were 9 cases of biopsy-proven presence of noncaseating epithelioid cell granulomas and the stain and the culture for acid-fast Bacilli and fungi were negative. 5 patients met criteria for the diagnosis of systemic sarcoidosis. 3 of the 4 patients who did not have a diagnosis of systemic sarcoidosis had a history of malignancy organ solid. We present these 3 cases in order to focus on sarcoid-like reaction in cancer patients.

Benefit of a second opinion: From metastatic disease to resectable lung cancer with sarcoid-like reaction
BACKGROUND Mediastinal lymphadenopathy in combination with lung cancer is suggestive for lymph node metastases but can also have other origins. CASE REPORT We describe a patient diagnosed with stage IV lung cancer presenting with parenchymal lesions and enlarged mediastinal lymph nodes. A second opinion including FDG-PET scan review and a mediastinoscopy followed by surgery revealed tumor spe...
متن کاملSarcoid in cancer patients: clinical characteristics and associated disease status.
BACKGROUND The increased risk of cancer in patients with sarcoidosis is well established, but there is little information regarding sarcoid diagnosed during or following cancer. The purpose of this study is to describe and analyze the disease status and events associated with the diagnosis of sarcoid in cancer patients. METHODS At a large cancer center, we identified 64 patients with a pathol...
متن کاملSarcoid reaction associated with Merkel cell carcinoma revealed by fluorodeoxyglucose positron emission tomography: a case report
Introduction: Although the association between cancer and sarcoidosis or sarcoid reaction is known, sarcoid reaction associated with Merkel cell carcinoma is rare. Case presentation: We report the case of a 57-year-old Japanese woman with Merkel cell carcinoma in the inguinal area associated with sarcoid reaction. Fluorodeoxyglucose positron emission tomography demonstrated elevated fluorodeoxy...
متن کاملClinical implications of granulomatous inflammation detected by endobronchial ultrasound transbronchial needle aspiration in patients with suspected cancer recurrence in the mediastinum
BACKGROUND Granulomatous inflammation has been previously reported in association with cancer. Endobronchial ultrasound guided transbronchial needle aspiration (EBUS-TBNA) is a new minimally invasive test for investigating mediastinal lymphadenopathy. The identification of granulomatous inflammation by EBUS-TBNA and the clinical implications of such detection in a series of patients with previo...

The aim of this study was to analyze the clinical, computed tomography (CT), and positron emission tomography (PET) findings of sarcoidosis, sarcoid reaction, and malignant lymph nodes (LNs) to the results of transbronchial LN aspiration and biopsy (TBNA).The TBNA results of mediastinal and hilar LNs of 152 patients in our hospital from July 2008 to March 2013 were retrospectively reviewed. Two...
